Admission Packet

The following documents must be completed and on file before your child begins care. Contact our director at 304-757-9165 to confirm availability and get started.

  • Apply

    Complete a comprehensive application so we can ensure TVCDC is the right fit for your family.

  • Physician's Report

    Provide a report from your child's doctor detailing their current health status.

  • Immunization Certificate

    A WV provisional certificate of immunization is required by state regulations prior to enrollment.

  • Emergency Form

    Complete a notarized emergency form providing detailed contact information for your child's safety.

  • Food Reimbursement Form

    Complete a food reimbursement eligibility form to participate in our Child Care Food Program

  • Birth Certificate

    Provide a copy of your child's birth certificate to be kept on file.
